
Family & Youth Community Support Services
Healthplus provides community support services to individuals
with emotional disturbances, substance abuse and/or mental
illnesses. Community support serves as a link to area resources
and services. Healthplus staff monitors, develops, and assesses
service effectiveness toward personal goals. All services are
consumer focused, utilize a team approach, and can be provided
to adults and children in the community, home, work or school
setting. Services are normally covered by Medicaid or other
insurance providers.
Community Support Services include:
+ mentoring for children in school, home and community
+ mentoring for adults needing assistance with life skills
+ coordination and provision of monthly treatment team
+ person-centered planning meetings
+ updates to PCPs when services or goals need to be changed
+ daily assessment of services
+ justification to insurance providers of why services are needed
+ feedback to family members/others involved in client treatment
+ scheduling, attendance, and feedback at IEP meetings in collaboration with teachers and school staff
+ assistance with social skills and activities
